Download The WPForms Plugin
If you haven’t already, you need to download the WPForms plugin for your WordPress website. All you have to do to create online forms with WPForms is to drag and drop the fields you want and hit publish.

Once you have the Signature addon activated on your website, the field optional will appear when you go to create a form.
Create Your Form Using A Signature Form Template
After adding the Signature addon, you will be able to create a signature form the same way you have created your other online forms using WPForms.
Here is where you have the free reign to do what you want. Head on over to the WPForms tab and click “Add New”.
Start with a blank form template and assign the form the name “Signature Form.”
From there just drag and drop the field you want just like how you would when creating other forms through WPForms.
Electronic Signature fields typically go at the very bottom of forms right after an agreement checkbox.
How many and the types of fields you add are all up to you. You can create your online signature form to your liking. WPForms is completely customizable and has a wide variety of fields to choose from. Create it how you want!
Configure & Test Your New Electronic Signature Form
Head back into your form and select settings. You want to make sure everything is properly set up before publishing on your website.
The “General” tab is where you will assign your form a title and description. It is also where you can assign specific styling classes.
Next, make sure your confirmation and notification settings are set up correctly. Most importantly, verify that you have the right email added for which you want to receive submissions.
Once you’re are finished hit save and then click embed.
Take the shortcode that is generated for you and copy it.
From there, head on over to the page or post you wish to display your new form on.
Using the shortcode you copied, paste the form on the section of your page where you want the form to be displayed.

When you finally publish your page or post with your new signature form, test it out on desktop, mobile and tablet.
You will notice that your website visitors can sign using their mouse or finger (if touchscreen).
